Risk of malignancy in adrenal incidentalomas

Less than 2% of adrenal incidentalomas are found to be adrenocortical cancers.

Factors that affect the estimated risk of malignancy include:

i. size <4cm 4-6 cm >6 cm ii. Hounsfield units <10HU 10-20HU >20HU

risk 0.1% 2.5% 20% risk 0% 0.5% >5%

iii. growth rate of >0.5-1cm/year

iv. urine steroid metabolomics
